Pre-Surgery Preparation for Gastric Sleeve: Liquid Diet and Tests
Proper preparation before gastric sleeve reduces surgical risk, facilitates laparoscopic surgery by decreasing liver size and accelerates recovery. Dr. César Hernández Elenes in Mexicali explains the complete preparation protocol.
Preoperative studies
Requested 2-4 weeks before surgery: complete blood count, blood chemistry (glucose, kidney, liver function, electrolytes), coagulation times, lipid profile, thyroid hormones, vitamin D and B12, HbA1c if diabetic. Electrocardiogram and chest X-ray. Abdominal ultrasound to rule out gallstones. In older patients or with comorbidities, echocardiogram and pulmonary function tests.
Multidisciplinary evaluation
Consultation with bariatric surgeon (clinical evaluation, procedure explanation, informed consent signing), nutritionist (preoperative and postoperative plan), psychologist (evaluation of mental preparation, realistic expectations, support), anesthesiologist (anesthetic risk evaluation). This integral evaluation reduces complications.
Preoperative liquid diet (2 weeks)
The last 2 weeks before surgery a very low-calorie diet is indicated (800-1000 kcal/day) predominantly liquid: protein shakes with lactose-free milk or water, fat-free broths, sugar-free gelatin, infusions, water. Goal: reduce the size of the fatty liver (common in obesity) so surgery is safer and technically easier. A voluminous liver hinders visualization and increases risk of injury.
Medications: what to suspend
Suspend 10-14 days before: non-steroidal anti-inflammatories (ibuprofen, naproxen, aspirin without cardiovascular indication), unprescribed anticoagulants, high-dose vitamins E and K, St. John's wort. Continue habitually: antihypertensives, antidiabetics (adjusted by endocrinologist), thyroid hormones. Bring complete list to anesthetic evaluation.
Hygiene and specific care
Shower with antibacterial soap the night before and the morning of surgery. Do not apply creams, makeup or nail polish. Remove jewelry, piercings, contact lenses. Absolute fasting (no food or liquids) from 8 hours before. Long hair tied up. Use comfortable clothing for hospital arrival.
Previous habit changes
Stop tobacco completely at least 4 weeks before (reduces respiratory and healing complications). Reduce alcohol to minimum in the last 2 weeks. Incorporate daily walking (even briefly) to improve pulmonary capacity. Practice deep breathing techniques (incentive spirometry).
Logistical aspects
Organize companion during hospitalization (1-2 nights) and first week at home. Prepare food and supplements for postoperative diet (clear liquids, broths, gelatin, protein powder). Comfortable loose-waist clothing. If traveling from outside Mexicali, book accommodation for 5-7 days minimum. Identity documents and insurance authorization (if applicable).
Mental preparation
Informing yourself about the procedure and postoperative reduces anxiety. Talking with patients who already went through the experience helps. Maintain realistic expectations: gastric sleeve is a powerful tool but requires continuous commitment. Visualizing results at 6 and 12 months motivates in the short term.
