Plastic Surgery After Gastric Sleeve: When and Which Procedures to Consider

Dr. César Hernández Elenes||6 min read

After significant weight loss with gastric sleeve, many patients have excess skin that does not recover its natural elasticity. At Dr. César Hernández Elenes' office in Mexicali, we work together with certified plastic surgeons to offer the ideal complement to body contouring after bariatric surgery.

Why is there excess skin?

Skin has limited ability to contract after significant weight loss. When a patient loses 30-50 kg or more in 12-18 months, abdominal, arm, thigh and chest skin usually becomes flaccid. Factors that influence: age (elasticity decreases with years), magnitude of loss, original skin quality, smoking, nutrition and hydration.

When to consider plastic surgery

It is recommended to wait 12-18 months from gastric sleeve, with weight stabilized for at least 3-6 months. Performing plasty before can result in suboptimal results if the patient continues losing weight. Stable weight ensures aesthetic results are durable.

Most requested procedures

Abdominoplasty (abdominal wall repair and removal of skin apron) is the most common. Followed by: brachioplasty (arm remodeling), thighplasty (inner thighs), mastopexy with or without implants (breast lift), facial and cervical lift. Many patients require combinations: for example, abdominoplasty + brachioplasty at separate times.

Nutritional considerations before plasty

Before post-bariatric plastic surgery it is important: adequate protein (1.5-2 g/kg/day), vitamin and mineral supplementation (especially iron, B12, zinc, vitamin C), normal hemoglobin level, no smoking at least 4 weeks before and 4 weeks after, BMI preferably below 30 to reduce healing complications.

Risks and recovery

Post-bariatric plastic surgeries have higher risk of wound complications (seromas, dehiscence, slow healing) than standard plastic surgeries. Recovery times vary: 3-4 weeks for abdominoplasty, 2-3 weeks for brachioplasty, 4-6 weeks for stable visible results. Scars are definitive; located in strategic areas to minimize visibility.

Costs and recommended order

Typical procedure order: first abdominoplasty (greater aesthetic and functional impact), then brachioplasty or mastopexy, finally thighplasty if necessary. Some procedures can be combined in the same surgery. Costs vary by extent and complexity; in Mexicali prices are significantly lower than in the United States while maintaining comparable quality.

Comprehensive plan with Dr. Hernández

From the initial bariatric consultation, the patient's "complete journey" is considered: weight loss with gastric sleeve, weight consolidation at 12-18 months, and then evaluation for plastic surgery if desired. This planning allows anticipating times, costs and realistic expectations.
